Tattoo Description. Please include any words with correct spelling or dates to be added.

Age requirements for tattoos are 16yr or older, under 18yr must have parental consent.

Size Estimate in inches or equivalent to a common object.

Do you want the tattoo in black and grey/color or just pops of color?

If you plan to cover/add to or work around exsiting tattoos you must include a photo of your existing tattoo. The photo must be straight on, no blurry or angled photos. No videos.
